Submitting a Help Desk Request

Please submit a detailed email or phone message with as much of the following information as possible. If you do not know all of this information, do not hesitate to contact us with as much information as you can.

  • Name and/or Student System UserID, Department, and Job Function
  • Which database were you using? (e.g. Prod, Rept)
  • Date and time of incident/problem
  • What type of workstation are you using? (PC or MAC)
  • Which Operating System are you using? (Windows XP, MAC OS, Linux, etc.)
  • Which browser (and version) are you running? (IE 6, Firefox 1.0, Netscape 7.2, etc)
  • What were you doing when the problem occurred? (Data Entry, Query, etc.)
  • Have you used this function successfully in the past?
  • What is the navigation path to the page you were in?
  • Did you get an error message?
  • Did you take a screen shot of the error message?
