- Name and SES UserID, Department, and job function
- Which database were you using? (e.g. Prod, Rept)
- Date and time of incident/problem
- What type of workstation are you using? (PC or Mac)
- Which Operating System are you using? (Windows XP/Vista/7, Mac OS, Linux, etc.)
- Which browser (and version) are you using? (IE 8, Firefox 5.0, etc)
- Have you used this function successfully in the past?
- What is the navigation path to the page you were in?
- Did you get an error message?
- Can you take a screen shot of the error message?
